Examination: MRI OF THE RIGHT WRIST

CLINICAL HISTORY: 39 -year-old female with history of scaphoid fracture and wrist pain

MRI TECHNIQUE: Coronal GRE T2, PD, axial PD, FIR, coronal T2 and sagittal PD images of the right wrist were obtained on a 0.2 Tesla open MRI.

MRI FINDINGS: The proximal pole of the scaphoid is not well visualized and appears deformed. There is abnormal T2 hyperintense signal throughout the scaphoid and the lunate. Radiocarpal, intercarpal and carpometacarpal alignments are maintained. There is no gross disruption of the scapholunate ligament or triangular fibrocartilage complex. The lunotriquetral ligament is not visualized. Dorsal extensor tendons are intact. Flexor tendons in the carpal tunnel are preserved and there is no mass in the carpal tunnel. There is effusion in the distal radioulnar and intercarpal joints

IMPRESSION:

Findings are suggestive of fracture scaphoid. No evidence of avascular necrosis at this time, but this maybe beyond the resolution of the scanner, and a CT or higher resolution MRI would be more helpful.

Abnormal signal involving the lunate bone, likely representing bone bruise/bone edema; subtle fracture cannot be excluded
