Examination: MRI OF THE LEFT ANKLE

CLINICAL HISTORY: 42-year-old male with a history of anterior joint pain

MRI TECHNIQUE: Sag T1, FIR, coronal T2, PD and axial T2 PD images of the left ankle were obtained on a 0.2 Tesla open MRI.

MRI FINDINGS: There is no fracture or osteonecrosis. There is moderate thickening/scarring of the deep fibers of the deltoid. The anterior talofibular ligament is thinned, likely from remote injury. The anterior and posterior tibiofibular, calcaneofibular and posterior talofibular ligaments are intact. There is moderate insertional tendinosis of the tibialis anterior with a small intratendinous ganglion cyst. The extensor tendons are otherwise intact. There is a near full thickness tear of the peroneus longus tendon as it passes over the peroneal tubercle. The peroneus brevis is intact. There is moderate to severe insertional posterior tibial tendinosis with resultant pes planus. The flexor tendons are otherwise intact. A bilobed ganglion cyst overlying the extensor digitorum tendons is compressed centrally by the extensor retinaculum. Achilles tendon and plantar fascia are unremarkable

 
IMPRESSION:
 

Bilobed ganglion cyst overlying the extensor digitorum tendon, just deep to the extensor retinaculum.

Near full thickness peroneus longus tear at the level of the peroneal tubercle.

Moderate to severe insertional posterior tibial tendinosis with pes planus
