Preliminary Report
 

CT scan of the abdomen with intravenous contrast

Findings:

There is a complex anteroinferior splenic laceration. There is a perisplenic hematoma measuring approximately 3.3 x 2.3 cm with focal contrast blush, suggestive of active hemorrhage (image 49). There is also small amount of hemoperitoneum in the left paracolic gutter. The other abdominal viscera to the extent visualized are unremarkable. No free air. The lung bases are clear. No fracture is seen in the visualized osseous structures.

Impression:

Findings consistent with a complex splenic laceration with perisplenic hematoma demonstrating a focus of active hemorrhage as described.
Small amount of hemoperitoneum in the left paracolic gutter.
Recommend clinical correlation and further evaluation with CT pelvis as clinically indicated.
