Please call or fill out the request form to schedule an appointment. Feel free to leave a detailed message. I understand that your time is valuable and I will endeavor to get back to you as quickly as possible. Treatment by a physical therapist in Pennsylvania requires a current prescription from a licensed physician, osteopath, or dental practitioner. Treatment sessions are scheduled for half-hour or full-hour periods of one-on-one treatment and instruction. Your appointment is scheduled specifically for you. Please give 24 hour notice of cancellations; otherwise it will be necessary to charge you the full rate for the missed visit. Payment is due at the time of service. I do not accept third-party payers. I will provide you with a submittable invoice after every session. This will enable you to seek reimbursement from your insurer. These services are covered by most plans.
